You do not need to prove what is causing your symptoms before asking for help. The aim is to listen, assess what is happening and find ways to help you feel better.

Create your own account or sign in. You can record medicines and supplements, the amounts and times you actually used them, symptoms and any discomfort. You can also record bedtime, waking time, estimated sleep, exercise, activities, rest and how you feel.
Every new entry starts as “Only me”. Keep it private or mark selected entries to share with people you approve. You can pause all sharing or revoke someone’s access. This cannot remove copies they already saved.
A brief note is enough. You do not have to complete every section or repeatedly check your body. There are no compulsory sleep or exercise targets. The calendar shows your entries; it does not book appointments or send medicine reminders.
Nobody monitors this journal for emergencies. Writing an entry does not automatically notify your family or a doctor. The journal does not prescribe treatment or change medication.

Distressing sensations or perceptions, memory problems and major sleep changes need an in-person assessment. Several causes are possible, including effects of some medicines. A clinician needs to assess your own situation. [1, 2]
You could say: “I am struggling and need someone to come to an appointment with me. I would also like us to review everything I am taking.”
Do not try to remove anything with tweezers, needles or cuts. Do not use insecticides, bleach or irritating remedies. Do not put objects or products into your eyes, ears or nose. This can cause injury. Severe eye pain or changes in vision after an injury need urgent care. [3, 4]
Gather medicines, supplements, drops, prescriptions and medical reports. Note what you actually used and when. Do not change treatment because of this page. Ask a clinician or pharmacist to review it, especially if you are getting worse. [1]
A brief note is enough for organising information: what you felt, how much you slept and when it happened. You do not need to spend the whole day searching for explanations.
Choose who you want beside you and who may see your documents. Your family can help organise information, but should not decide a diagnosis by voting or hide treatment changes from you.
A message for your family could be: “Please listen without arguing, come with me and help me discuss this with a clinician.”
